Latest 4.5 release fails on domain upgrade in SQL Azure - nonclustered index

The previous releases didn't have this problem. But now, when DO tries to drop and re-add Int64-Key-Generator table, it fails with an error that Sql Azure doesn't support nonclustered indexes.
